The Primary Care Patient Care LPN will work alongside patients, providers, and other clinical team members to enhance quality outcomes for every individual. Position Overview: Experience: A minimum of 6 months in a clinical healthcare setting, ideally within a primary care environment. Education: Completion of an accredited practical nursing program is required. Licensure: Must hold a valid LPN (Licensed Practical Nurse) license in the state of practice. Additional Skills: Ability to function effectively as part of a team and collaborate with all care team members. Strong written and verbal communication skills are essential. A customer service mindset, along with excellent organizational and interpersonal abilities, is necessary.
