Job Title: Case Manager Profession: RN (Registered Nurse) Specialty: Care Coordination/Case Management Duration: 13 weeks Shift: 8H Days Hours per Shift: 8 hours Experience: Minimum 2 years License: Required Certifications: BLS Certification Must-Have: - Prior experience in care coordination or case management. - Familiarity with Interqual criteria. - Ability to collaborate and work effectively in a team. - Strong communication skills. - Ability to reprioritize tasks throughout the day. Description: This position involves case management and care coordination. The role requires dealing with various payers, including uninsured and homeless patients. Responsibilities include completing initial assessments, planning for discharge, and utilization review. Candidates will be required to manage 15-20 patients or consults per day. There is a weekend requirement every other week, with at least one full weekend per month. Holidays are not mandatory, but willingness to work them is appreciated. Orientation for new hires lasts five days, focusing on care coordination. Candidates must be willing to train in utilization management and discharge planning.
