Job Title: Case Manager RN - Home Health Profession: Registered Nurse Specialty: Case Manager Duration: 13 weeks Shift: Day Hours per Shift: 8 hours Experience: Minimum of 2 years required License: CA RN License Certifications: BLS, Oasis documentation experience, EPIC experience Must-Have: - Home health experience as a case manager is required. - Recent experience in home health is necessary, not hospice. Description: The experienced registered nurse is responsible for the overall management of patients, including the provision of direct care while meeting designated productivity standards. This position plans, organizes, and directs all patient care services for patients in the assigned caseload while ensuring optimal quality of care. Responsible for coordinating the interdisciplinary team to implement the established plan of patient care while managing visit utilization and expenses effectively. Engages in continuous development in professional nursing practice. Key Responsibilities: Conduct comprehensive patient and family assessments that encompass physiological, spiritual, and emotional areas. Collaborate effectively with physicians and other healthcare team members to interpret and adjust in-home patient care plans. Partner with peers and other healthcare providers to streamline patient workflow and improve outcomes. Communicate the patient plan of care clearly to coworkers, physicians, and other departments.
