The Physical Therapist will evaluate, develop, and implement individualized treatment plans. The goal is to improve mobility, strength, and overall functional independence. SNF experience is preferred, but mentorship is available for candidates with limited experience. The position requires collaboration with the interdisciplinary care team. Documentation must be completed in accordance with facility standards. The position may require floating to a nearby facility. Holiday coverage is required.
